Agreeing a care plan

Find out about agreeing a care plan - an important part of getting the most from your care provider.

When you first meet your care workers, they should spend time getting to know you and then agree a care plan that you are happy with.

The care plan will set out your needs and how staff will meet these needs. The plan should include your social, personal and health needs and how these will be met.

Have your say

The care plan is a document written with your input and should be reviewed regularly to make sure your needs are being met.

Your care workers should let you know what they plan to do for you, when, and how. You have a right to expect them to listen to you.

However, if you never tell them what you would like them to do, they will never know.

They should also regularly ask you whether you are happy with the care you get and if they could do anything to improve the way they work.
