Improved AQAA forms now available

Updated versions of the annual quality assurance assessment
(AQAA) self-assessment and dataset forms are now available from
this website.
We have made improvements to the forms based on the feedback you
have given us.

What's new
We have made a number of changes following comments and
suggestions about previous versions. Some questions have been
removed, some have been changed, a few have been added. Where there
are new questions the guidance explains why.
Workforce/Skills for Care (SfC) data
Almost all of the workforce questions are now in an Appendix to
the AQAA.
If you have completed both parts of the SfC National Minimum
Dataset for Social Care (NMDS-SC) you do not need to fill in the
Appendix. Instead, go to the NMDS-SC-Online website and
download the NMDS-SC Workforce report for your service.
You can send that report to us, with the rest of the AQAA, and
leave the Appendix blank.
Note: The AQAA for adult placement schemes does
not have an Appendix for workforce questions because carers provide
the direct care. SfC do not collect information about carers. So,
providers of adult placement schemes will not be able to download a
Workforce report from NMDS-SC-Online.
Diversity
The guidance explains why this information is important. It also
makes clear that we do not expect providers to survey people who
use services about diversity in order to fill in the AQAA.
Word processing
We are still providing unlocked versions for providers who use
different word processing software. This should enable you to ‘cut
and paste’ into your AQAA using your own software.
Important: you must make sure you haven’t
deleted or altered any questions before sending the form back to
us.
