Notifications: guidance and forms

We have produced new guidance and forms to improve
the way we handle statutory notifications from
providers.
This new methodology will help us to work with providers more
effectively using IBL principles.
The guidance tells you what you have to notify
us about by law and what we will do with your notification.
We will record the numbers of routine notifications, such as
expected deaths, and only refer significant problems on to our
inspection and registration teams.
There are two forms for each kind of service (two each for care
homes, agencies and adult placement schemes)
- the first for sending us notifications about people who use the
service
- the second for sending us notifications about the service’s
premises, registered provider(s), registered manager, staff, or
statement of purpose
You do not have to use the forms, but if you do they
will help you to make sure that you have told us everything the law
requires. They also provide a space for you to tell us what you
have done about the matter. This will help us to decide whether you
have handled it correctly.
If you complete the form on a computer you can move from text
box to text box by pressing your ‘Tab’ key. You can put ‘X’s in
check boxes by pressing your spacebar.
Sending notifications that include personal information by email
or fax does not comply with the Data Protection Act 1998. Most
notifications include personal information. You should therefore
send your notifications ONLY by post. Please do not send the same
notification by post and email or fax. In all
cases only send each notification once.
